EJ-130
Writ of Execution
Issued by the court clerk after judgment. A levy must be made within 180 days after the writ is issued.

MC-012
Memorandum of Costs After Judgment, Acknowledgment of Credit, and Declaration of Accrued Interest
Claims the costs of enforcing the judgment, credits for payments received, and accrued interest, so they can be added to the amount due.

WG-015
Declaration of Address Verification (form WG-015/EJ-135)
Required for judgments for personal debt. Give it to the levying officer and file it with the court within 10 business days.

Request for Sheriff to Serve Court Papers
The required instructions to the sheriff for a writ or levy. Not used for wage garnishments.

Application for Earnings Withholding Order
Filed with the levying officer to start a wage garnishment, in place of SER-001.

Earnings Withholding Order
The order the levying officer serves on the employer, directing it to withhold part of the debtor's earnings.

Employee Instructions

EJ-150
Notice of Levy
Served on the bank or other third party holding the debtor's property, and on the judgment debtor.

EJ-160
Claim of Exemption (Enforcement of Judgment)
Used by a judgment debtor to claim that levied property, such as money in a bank account, is exempt. It is filed with the levying officer.

Witness Fee Calculator
A witness subpoenaed to appear is entitled to a daily fee plus mileage for the trip both ways. Enter the one-way distance and the number of days to estimate the fee.
California: $35.00 daily fee + $4.00 mileage (20 miles at $0.20) = $39.00
- California civil cases: $35 a day plus 20 cents a mile, both ways (Government Code section 68093).
- Federal cases: $40 a day plus mileage at the General Services Administration rate for a privately owned car, which rose from 72.5 cents to 76 cents a mile on July 1, 2026 (28 U.S.C. section 1821). The GSA usually sets the rate each January, so check it for travel in 2027.

Are these the current versions of the forms?
Yes. Each court form links to the California Courts website, which always has the current version. The revision dates on this page were checked in October 2026. The Judicial Council revises forms every January 1 and July 1.
Why link to the court's website instead of hosting the forms?
Courts can reject a filing on an outdated form. Linking to the official file means you always download the current revision.
How much is a witness fee in California?
For civil cases in superior court, $35 a day plus 20 cents a mile, both ways (Government Code section 68093). In federal court it is $40 a day plus mileage at the federal rate, 76 cents a mile since July 1, 2026.
